Medicinal Fungi Evidence Update

Surveillance window: 23–30 October 2026 | Human evidence and integrity watch

“Two human trials merit manuscript attention, but neither supports a broad lion's-mane or Reishi claim. One accepted peer-reviewed lion's-mane trial was positive on a single attention/working-memory task while its composite and most cognitive tests were null. A second trial found a lion's-mane/Reishi blend better than lion's mane alone on its primary anxiety measure, but not demonstrably better than placebo. Two retractions require immediate citation checks.”

What changed

Two human trials merit manuscript attention, but neither supports a broad lion's-mane or Reishi claim. Two retractions require immediate citation checks.

Human clinical evidence(3)

Newly indexed human evidence is assessed against the exact formulation and study design, without transferring results to a mushroom species or product category.

Lion's mane for subjective cognitive difficulty

Tested intervention

2 g/day in three vegetable-cellulose capsules of certified-organic powdered Hericium erinaceus strain M2-102-10. The preparation combined fruiting body and mycelial biomass grown by controlled solid-state fermentation on certified-organic whole oats, then dehydrated and milled. Comparator: visually matched capsules containing 2 g acacia gum. This is not evidence for fruiting-body-only powder, an extract, isolated erinacines/hericenones, or lion's mane generally.

Status and design: accepted peer-reviewed Frontiers clinical-trial article; final formatted full text is pending. The corresponding April preprint and registry provide the detailed methods. Remote, randomised, double-blind, placebo-controlled, eight-week trial in adults aged 40-75 with self-reported mild-to-moderate cognitive difficulty.

Population and analysis: 150 were randomised; 120 completed; 109 were analysed (57 active, 52 placebo). The gap between randomised and analysed participants is substantial. The registry was first posted on 25 March 2025, after the 21 February study start, and describes an observational, one-treatment-group study even though the article reports a two-arm RCT.

Outcomes: the Juggle Factor task improved more over time with the product (p=0.001; +0.45 z-score), but no individual timepoint separated significantly. Memory Grid, Target Tracker, Mental Map, Sandia's Matrices and the composite cognitive score were null. Subjective sleep quality, morning restedness and mood improved faster; placebo improved motivation faster. No adverse events occurred in the active arm.

Interpretation and bias: one task-specific signal amid several null tests does not establish broad cognitive enhancement. The registry names a composite attention/short-term/working-memory outcome, while the paper foregrounds one component despite the null composite. M2 Ingredients funded the study, supplied the product and employed two authors; sponsor authors reviewed the manuscript.

Manuscript verdict

Change now. Add this exact M2-102-10 preparation as a mixed result: one positive task, null composite and most cognitive tests, sponsor funding, attrition and registry/reporting mismatch. Do not upgrade the clinical grade.

Lion's mane/Reishi blend for anxiety and wellbeing

Tested intervention

Three 600 mg capsules each morning (1.8 g/day). SO-DSX1 was a proprietary organic H. erinaceus complex; supplier materials describe fruiting body plus mycelium grown on organic sorghum. SO-DSX2 contained 60% organic H. erinaceus complex and 40% Ganoderma lucidum complex, standardised to 20% beta-glucans — approximately 1.08 g and 0.72 g/day respectively. Placebo was microcrystalline cellulose. Fungal strains, the precise tissue balance and a complete chemical profile were not reported; these were powders, not identified solvent extracts.

Status and design: peer-reviewed, published 20 October and identified after the previous brief. Randomised, double-blind, placebo-controlled, three-arm trial in 84 women aged 18-26 who felt stressed and/or anxious but had no diagnosed psychiatric disorder. Arms were placebo n=28, blend n=27 and lion's mane n=29; treatment lasted 28 days with a Day-42 follow-up.

Primary outcome: the blend produced lower GAD-7 anxiety than lion's mane overall (p=0.018) and on Day 28 (p=0.003). The article reports no significant blend-versus-placebo contrast for the primary outcome. Thus it does not establish placebo-controlled anxiolytic efficacy. Lion's mane alone showed no advantage.

Secondary and null findings: HADS anxiety and perceived-stress interactions did not survive false-discovery-rate correction (q=0.072). Stress VAS, fatigue and self-esteem showed no treatment effect. All groups improved substantially across every psychological measure, consistent with participation, expectancy or repeated-assessment effects. Adverse-event frequency did not differ; 50 events occurred in 31 participants, with one placebo withdrawal.

Quality and bias: good blinding and 96.7% capsule-count compliance. However, recruitment stopped at 84 versus a 135 target; baseline fruit/vegetable intake, waist-to-hip ratio and stress VAS differed; the active arms contained unequal lion's-mane doses; and no Reishi-only arm can separate Reishi from mixture or dose effects. Northumbria funded the study internally; Sempera Organics supplied products and was reported to have no research role; authors declared no conflicts.

Manuscript verdict

Add a caveat/watch note. Record the blend-versus-lion's-mane finding, but state explicitly that primary superiority over placebo was not demonstrated and secondary signals failed multiplicity correction. No grade change.

Systematic reviews and meta-analyses: none qualifying

No new systematic review or meta-analysis met the inclusion threshold. A 23 October narrative review of Cordyceps taxonomy, metabolites and production usefully emphasises species authentication, product standardisation, pharmacokinetic uncertainty and inadequate clinical validation, but supplies no pooled clinical estimate and should not change an evidence grade.

Manuscript verdict

No change. Optional supporting citation for the product-identity and translation-caveat sections.

Laboratory and animal signals(1)

Signals that may justify monitoring or further investigation, but do not establish a human clinical effect.

Quality-filter exclusion: under-characterised lion's-mane polysaccharide in mice

Tested intervention

300 mg/kg/day of a material called H. erinaceus polysaccharide for 42 days in male C57BL/6J mice on a high-fat/high-sucrose diet; metformin 300 mg/kg/day was the positive comparator. Fungal strain, source tissue, extraction/purification, composition, molecular weight and supplier were not reported.

No newly published preclinical study cleared the high-quality threshold this week.

This peer-reviewed 27 October mouse paper reported improvements in metabolic measures, Y-maze alternation, barrier proteins and hippocampal inflammatory/neurochemical markers.

Overall group sizes and blinding are not stated; selected analyses used six mice per group and western blots used three biological samples. Microbiome changes are associative: there was no transfer, depletion or pathway-blockade experiment. Public Chinese grants funded the work and no commercial conflicts were declared.

Manuscript verdict

No change. Do not use this under-characterised single-dose mouse study to support cognition, metabolic or gut-brain claims for lion's mane products.

Integrity watch

Replication, null and correction watch

Human null pattern: in the M2-102-10 trial, the composite and five named cognitive tests did not beat placebo; in the young-women trial, lion's mane alone showed no advantage and the blend did not show primary superiority over placebo. These are more informative than the positive headlines.

Status transition: the M2-102-10 study is no longer merely a preprint; an accepted peer-reviewed article now exists, although the final formatted full text remains pending. The result remains narrow rather than broadly confirmatory.

Retraction — change required: the 2017 rat-diet paper claiming that Pleurotus tuber-regium ameliorated carbon-tetrachloride reproductive/testicular injury was retracted on 26 October. The editor cited apparent image duplication/resizing, unavailable original images and missing ethics-approval documentation, and lost confidence in the data. Manuscript: change now. Remove the 2017 P. tuber-regium paper wherever cited and reassess any dependent claim. Source: Retraction note, Clinical Phytoscience. doi:10.1186/s40816-026-00448-y

Late integrity catch — change required: the 2022 ganoderic-acid-A paper on TLR4/NLRP3 signalling in intervertebral-disc degeneration was retracted online on 1 October. The publisher cited significant data-integrity concerns that the authors' original data and responses did not resolve. Manuscript: change now. Remove the 2022 ganoderic-acid-A/IVDD paper if present and downgrade any mechanism relying on it. Source: Retraction statement, Bioengineered. doi:10.1080/21655979.2026.2730725

Manuscript verdict

Change now: remove the two retracted papers wherever cited. Clinical grades: no changes.

DecisionAction
Change nowAdd the M2-102-10 trial as a mixed task-specific result: one positive Juggle Factor trajectory, null composite and most cognitive measures, sponsor funding, attrition and registry mismatch. No grade upgrade.
Change nowRemove the retracted 2017 P. tuber-regium reproductive-toxicity rat paper and the retracted 2022 ganoderic-acid-A/IVDD paper wherever cited.
Add caveat / watch noteFor SO-DSX2, record lower GAD-7 versus lion's mane alone, but no demonstrated primary advantage over placebo; secondary HADS/PSS signals failed FDR correction.
No changeDo not promote the under-characterised HEP mouse paper into claim-supporting evidence. No qualifying systematic review appeared.
Clinical gradesNo changes.
Broad mushroom claimsNo changes. Every conclusion remains tied to the named preparation, dose, population and comparator.

Editorial takeaway: the week strengthens the book by narrowing claims and cleaning the record. The best new human signal is one task within one sponsor-funded preparation; the strongest immediate actions are two retraction removals.
